If you are not able to join us, there is no need to complete this form UNLESS you would like to make a freewill donation toward Bishop John's Purse. (See next section.)

Bishop John’s Purse

Not only is this evening be an opportunity to show our faithful support of Bishop John and his diocese, it also will be a chance to offer our financial support.

If you wish to make a freewill donation of any amount toward Bishop John's Purse:

1. Select the "Custom Amount" button on the form on the left.

2. Calculate the TOTAL amount of your payment -- $50/per attendee PLUS the dollar amount you would like to donate toward the Bishop's purse -- and type that TOTAL amount into the box that appears. 

Your purse donation will be calculated and properly directed based on the attendee information you provide elsewhere on the form.

To RSVP/Donate by Check Instead:

  1. Please notify us by email at ubeaz.exec@gmail.com that you will be sending a check and tell us: (A) The name(s) of the attendee(s); (B) Any food allergies or dietary restrictions; and (C) Whether the attendee(s) will be riding in the motorcoach.

  2. Calculate the TOTAL amount of your payment -- $50/per attendee PLUS the dollar amount, if any, you wish to donate toward the Bishop John's purse.

  3. Make your check payable to “UBE Arizona” for the TOTAL amount, and mail it to:
